Sincere Residential provides authorized Indiana Medicaid waiver services and participates in Indiana PathWays for Aging. We also offer private-pay home care. Medicaid waiver services require eligibility, an authorized service plan, and coordination with the participant’s case manager, service coordinator, or managed-care plan. 

Medicaid Waiver services provide in-home care and support for eligible individuals, helping them remain at home instead of moving to a facility. Indiana Medicaid home and community-based waiver programs help eligible individuals receive approved support in their homes and communities instead of institutional settings. Eligibility, services, hours, and funding are determined through the applicable waiver program and person-centered planning process. 

Eligibility depends on the applicable waiver’s financial, functional, age, disability, and level-of-care requirements. Sincere Residential cannot determine eligibility, but we can explain our services and help connect families with the appropriate state agency, health plan, Area Agency on Aging, or case-management resource. 

Sincere Residential serves Indianapolis and surrounding Central Indiana communities. Service availability depends on the client’s location, requested schedule, authorized services, and current staffing. 

 Yes. Families may arrange Personal & Attendant Care and Homemaker Services directly through a private-pay service agreement. We explain the proposed schedule, rates, payment arrangements, and service plan before care begins. 

In some circumstances, an eligible family member may provide paid care through an authorized Indiana Medicaid waiver service. Eligibility depends on the participant’s waiver, approved services, caregiver relationship, employment qualifications, and person-centered plan. Compensation is not guaranteed, and services must be authorized before paid care begins. 

No. Eligibility, authorized services, service hours, and funding are determined by Indiana Medicaid, the applicable waiver program, managed-care entity, and planning team. Sincere Residential provides only properly authorized services. 

Contact Sincere Residential and speak with your case manager, service coordinator, or managed-care plan. With appropriate permission, we can coordinate with your support team to review authorized services, staffing needs, location, and provider availability. 

Start times vary based on assessment requirements, service authorization, documentation, location, schedule, caregiver onboarding, and staffing availability. We will explain the expected process after reviewing your situation. 

 Caregivers must satisfy applicable background-screening, orientation, training, competency, documentation, and employment requirements. Requirements may differ according to the service and funding program. 

Yes. Sincere Residential uses a person-centered approach that respects the individual’s choices, routines, goals, culture, communication preferences, and authorized support plan. Families and guardians may participate when requested or legally appropriate. 

Sincere Residential provides non-medical in-home and waiver support services. The website and contact forms should not be used for emergencies. Call 911 for an emergency and contact an appropriate licensed healthcare professional for medical advice.
